Pregnancy care in BC: choosing a midwife, family doctor or obstetrician, and the MSP wait period

Prenatal care in BC can be led by a midwife, a family doctor or an obstetrician, and MSP pays for one. Finding care without a family doctor, screening dates, and the MSP wait-period cost risk.

Three kinds of providers can lead prenatal care

Not having a family doctor does not stop you from getting prenatal care. The main provider for prenatal care and birth can be a midwife, a family doctor who does obstetrics, or an obstetrician. Obstetricians generally take higher-risk referrals, such as twins or type 1 diabetes. MSP pays for one of them, from pregnancy through 6 weeks after the birth. [1]

Midwives: fully covered by MSP and open to self-referral

Midwifery care is fully covered by MSP and needs no referral; you can call as soon as you know you are pregnant. If a practice is full, try several, since spots open up when people withdraw. About 70% of births attended by midwives take place in hospital, and over a quarter of babies in BC are delivered by midwives. [2][3]

How to find care without a family doctor

  • South Community Birth Program (on Kingsway) accepts self-registration. [4]
  • BirthDocs, the family-physician maternity team at BC Women's Hospital.
  • The PregnancyVancouver website lists family doctors who provide obstetric care. [5]
  • Call 811 and a navigator can help you find care.

Common prenatal timing

Before 10 weeksDiscuss prenatal screening with your provider
11 to 13 weeksUltrasound to confirm dates
17 to 22 weeksDetailed anatomy ultrasound
24 to 28 weeksGestational diabetes screening

BC Women's Hospital lists people of East Asian ancestry as being at higher risk of gestational diabetes, so testing may be offered earlier. [6] Non-invasive prenatal testing (NIPT) is publicly funded only in cases such as a higher-chance screening result; otherwise it is paid for privately. [7]

The cost to plan for: the MSP wait period

For new arrivals, MSP starts after the rest of the month of arrival plus 2 full months. The government page notes that most private insurance does not cover pre-existing situations such as pregnancy. [8] Without MSP, a hospital stay at BC Women's Hospital starts at several thousand dollars per day on the hospital's November 2025 rate list, with physician fees extra. [9] Anyone planning a pregnancy should build the wait period into the timeline.

Fees and programs change; check HealthLink BC, the BC government and hospital websites. In an emergency during pregnancy call 911; for non-urgent health questions call 811. [1][8]
